Service Linker is an online attendance register developed for people accessing community services in the Logan region of Queensland. It keeps a list of the different community services that a person visits, so that clients and workers can have a conversation about what assistance they are receiving and where. This can help everyone, both clients and workers, to get the best result for the person without wasting extra time and effort.
By registering your details in Service Linker when you visit a participating service, you can work more easily with the Case Workers of the different programs you attend to help you get the best outcome without being continually asked to repeat the same activity or re-tell your story.
How does it work?
When you visit a participating organisation in the Logan region, Service Linker asks a few simple questions such as your name, how old you are, where you live, and what program you are attending. A list of all the participating services in the Logan region you visited is then displayed.
You can then talk with your Case Worker about how your needs can be better met by all the services that you use, not just the one that you are attending at the time.
What questions will I be asked?
A person visiting the service will be asked to fill out an online form with:
- Personal Details such as your Name, Phone number, Gender, Suburb, Postcode and Email address
- Which program you are attending
You will then be provided with a list of any other participating programs that you have visited recently.
What happens to the information?
Your personal information is safely stored in an online database. Only authorised people, such as you or your Case Worker can view your personal information.
Managers at the service can access a list of all the visits recorded at their organisation, and what other types of organisations and services they visited. Names are removed from this list so that individuals can’t be identified. This information is important for organisations in Logan so that they can see what sort of services people are currently using, and what sort of services they need to provide for people in the future.
How can people participate?
If visiting a service, a person can:
- Talk to a worker about how Service Linker works and why it is useful
- Understand their right to Privacy
- Sign in to Service Linker for each service that they visit
